In all seriousness there are some genetic and / or hormonal factors that can make that happen. Without serious medical issues. And especially in Japan. Voice too.

@Aereus speaking of that, I found few really short manga which take that part a bit more seriously (where cross-dressers use techniques like tightening around belly using that old cloth while finding suitable boys for the trap plot) yet cutely but forgot the titles and are hard to find now. They may be translated by the honey group or some similar ones, clumsily shared in online reading web... but yeah, this is very rare.
