It's pretty much at the point where it seems like nothing concrete is going to be revealed until the third girl is brought into the story (it's pretty safe to say the third one isn't Hiba at this point).

Lunas face was always shown on the photo from the beginning to show it was her. When it was revealed Kawashina was the child from the photo, her face was revealed on the photo. Even after Hiba was introduced, the third face wasn't revealed. So I'm assuming she isn't the third girl from the photo.

My reasoning for that would be the chest size discrepancy between third girl from the past and Hiba in the present. But I didn't count her out just yet because this panel shot of her (pay attention to her hair) is not too dissimilar to the one in the picture, and the breast size difference could be explained either by pads, the picture was taken at a later date, or her choice of clothes makes her look slimmer than she really is.

What are "three S"?

The three "S" are Safe, Sane and conSensual; the most basic rule with affective and/or sexual fetishes. It's more widely related to BDSM, but it applies to basically all of them.
She's not respecting them because she goes too far with her biting, and I think she has dawn blood before (unsafe), she's being pretty obsessive about the MC (not-sane) and, this time at least, the MC permission on the act is shaky at best (non-consensual). Yeah, the bust size is one of the major reasons I don't think that it's Hiba in the photo, the girl in the photo seems to be even larger than Luna. In the present time in the manga, I don't think that would be easy to disguise or explain away.

Other than that, even though we've only seen Hiba once in the flashback, the style of clothing seems a bit too different from what she would wear (conjecture based on only what we've seen). Combined with that fact that the third girl is standing on the other side of Naoto when you would expect her to be standing with her friend if it was Hiba, it raises some doubts about it being her (a bit of a reach).

Unfortunately, long black hair is the norm, so it's pretty hard to use that to try and make the connection.

I'm still thinking the third girl (if it isn't Hiba) was older than the rest of them (maybe late teens, early 20's) and she is going to be some kind of milf character, or even a teacher (who knows at this point).

To be fair those rule can be disregarded in the moment if the perpetrator is hot enough or the victim is madly thirst inlove with the perp.

Well, if author is playing 5D chess, trying to make us think they are only baiting us, that would be worthy of praise.
There is still quite enough room for this to be only a bait, though.

She's probably either just into eroges or is a voice actress like most people speculated early on. It explains the condom if she doesn't know what sound it makes or something, and it explains the moaning sounds.

I mean like if the MC still likes the main girl and she's single, like what is he gonna do when he figures out she's been banging? Just give up cuz she's used good or something? It's just not normal behavior outside of fiction. Previous relationship can often bother people but most people have sex with more than 1 person through the course of their lives. They don't like combust because they dared to commit other relationships.

But this is a Japanese manga so most likely everything is a bait. I seriously can't imagine at this point that she somehow had a boyfriend that he's just missed seeing every single time despite living in the same freaking house.

Yeah, I mean, even if she were fucking a bunch of dudes for real, the fact is that MC already believes that... And he actually doesn't seem to care too much and still likes her.
But the fact the author didn't make MC have a negative reaction just for drama, but hasn't shown this supposed boyfriend that is fucking her eveyday, the reality may be that it's a misunderstanding... Or she actually has a girlfriend. I guess it could be an open relationship or something so still space for MC, who knows what the author is truly cooking.

Back in Chapter 4 the pages from here make it apparent that SHE is the one moaning (since she can't hold them back) and says "I don't want to hide anything from Nao-kun because... (queue flashback of her POV of the child him seeing him apparently kissing a guy, after which she appears slightly sad) when I hide something from Nao-kun he gets all sulky."

Of course we don't see who she's talking too (probably on either phone speaker or voice chat since we see no Bluetooth earpiece) but this sure makes it seem someone is making her moan. The eroge game wouldn't be making her moan unless she's masturbating to it and that's something I don't think she'd be discussing with a friend. This would also drive a nail in the coffin of the VA hypothesis because of COURSE you don't hide your moaning when that's the whole point, and if you're playing with toys on a livestream (in that filthy room?) then holding back sounds is also nonsensical.

An option that nobody has mentioned which is also off the wall is that maybe she is having submissive phone/chat sex with someone ordering her perform acts, but I think the dom would want to hear her too so there wouldn't be attempt at holding back either.

Just a reminder, there is this one time we're explicitly shown someone making Luna moan that isn't just Naoto's imagination of what's happening.

We have been shown that she's prone to do things that can look sus if it weren't for the viewers having prior knowledge of what's actually happening (See "ice cream scene", chapter 7, what would the scene look like if we didn't know she's just eating ice cream? If Naoto and the ice cream were not shown? What if something was blocking the view?). If you remove the knowledge that it is just Naoto using the electric massager on her, suddenly her moan bubbles are practically the same, albeit less intense because we now know Naoto is usually holding back when it comes to Luna, as when she's moaning out of view ("that spot", "do it more", "feels good"), especially in the raws. We could have been shown what's really happening behind the scenes already, we just didn't notice. But most will take it for something else entirely, because without the proper context, what else could it be?

Although, that doesn't explain why she had to wear a maid outfit that one time.

Then again, with a figure like hers, it's not out of the question for her to be doing some type of modelling as a job. They could just, for whatever reason, made her wear the maid outfit during the massage. Assuming Hiba's job has something to do with fashion/clothes design, then this theory could stick. But hey, that's just a theory.
