We need more context over his past than that same panel of her crying we've seen plenty of times.

Was it just ignore or abandonment issues because he worked himself to the bone over prioritizing them both as a married couple?

Even if he did or didn't do as previous question asked, what if he cheated with someone else (this is assuming that maybe several issues piled together)? Although Konishikawa seems honest enough not to casually cheat but we don't know the full truth.

Maybe and this is throwing a shot to the dark, a miscarriage we haven't heard about is the core issue (or part of their possible pile of issues)? At least from her wife's side it would be heavy enough and a reason for that recurring crying panel.
Yeah ... I have concerns. It was years ago, so anything could have happened and it will still be more important to judge ML by the man he is now, but that "anything" may ruin our (mostly) squeaky-clean image of him.
... But then again: rich, rich drama and additional depth for us to enjoy.
... But then again again: the sweet romantic euphoria of the MCs getting together could be much complicated and delayed, well beyond the steady slow burn it has been up to now. (And this manga doesn't have the most patient readers.)

iirc she wasn't his wife technically, they were engaged when they broke up.

You are correct, it slipped my mind that they were not married. However I will still assume everything I pointed out still has a possibility to apply.

They not even being married makes the situation even more interesting, how did they even reach that point with their relationship and why is he so concerned about it that he ended up like that romantically speaking. If not married why does it weigh heavily on him

Another instance of time (and convenient interruptions) moving extra fast in Manga Land.
In real life, people go out for dinner and drinks for around 5 hours, and I feel that two close male buddies could fully hash out a romantic issue like this in less than half of that -- and still have all the obligatory talk of work, family, and sports. Instead, ML and his guy get into the topic of Misato (the supposed "main course") for about 5 minutes, then suddenly it's "last orders" and closing time.
Still, a lot of emotional progress here for ML. Unfortunately that means other (external) events are about to get in the way.

We can't say how they got to that point, but there aren't many things I can imagine that warrant his ex saying something that venomous. That comment is haunting him how many years later? It's such a cruel thing to say.

You're absolutely right, which is why it's just so hard to imagine her saying it ONLY in response to ... [checks old chapters] ... his getting too wrapped up in work.
Well, we're gonna find out either way, folks -- Buckle in!

am i tripping or is the art style suddenly different
you sir/ma'am have fallen down the whole flight of stairs.

but fr fr 67, the art is mostly the same. I think the author pushed the style a bit more to the comedic side.

More importantly, we barely get to see Konishikawa make more faces than the few ones he does at the office. He's either playfully despairing, or being aloof. That can impact how we see the art!
