To be honest, I would imagine it was a lot easier to lose touch with childhood friends back before social media took root. Nowadays, it's easy to keep in contact with someone unless they were super young.

If not some plot twist with him having a twin bro or someone similar , if not her assuming he's what his future self would look like versus "the real childhood friend ended up as a shut in neet/hikkikomori/stereotypical chubby otaku loser archetype"

LOL it would be amusing if it's like "Fate keeps pushing together even if she doesn't actively seek him out" but if they live in the same city/she doesn't leave the area they probably would run into each other fairly often unless he's a total homebody outside of work
