Oof.

Don't think it's a romance thing. Just shows how siblings are always there for you throughout childhood, but when they get married they need to prioritize their spouse first, and that loss hurts.

I had to think about it for some moments. I made the assumption that there's the whole incest thing with Japan so it may be romance like @Randomizer said. But I feel it's more what @Megane2426 said. He's been dependent on his older sister all his life where she was always there for him. Now she's not always going to be there for/with him and that realization hurts. Like when a best friend moves far away forever.

It’s a very touching story - the boy who’s always dependent on his older sister and when she’s marrying away, she gently lets him know that she can’t continue to protect him - which finally brings him to the reality that things won’t be as they always were.

I mean, for the purposes of the general gist of the feeling here, it doesn't really matter either way.

That is to say—What exactly he felt about his sister could be construed either way (he's obviously unusually shaken up about her getting married, but that could be for any number of reasons; she might be moving far away to live with her new husband, for instance), but either way the main point is he's grown up emotionally dependent on her, and can't be anymore.
