In Yakuza terms, nee-san (sister) can be used to refer to a elder sister (doesn't need to be blood related) within the tribe. In the manga, he says "So you have a life to live too huh, I leave (our) sister to you" Which could mean he is leaving Nezumi to the care of Ao-Kun.

And finally we might be an answer to why Ao-Kun's sister was assassinated in the first place. Did she have ties to the underworld? Was Ao-kun brought in after her death and knew that Nezumi was the assassin and the org believed that he would not fall in love with her with Nezumi being his sister's killer?

Tbf, one of nezumi's previous targets was a literal elementary schooler so i'm sure if someone had enough money they'd just pay a yakuza to take out someone they dislike/an inconvenience to them as opposed to kidnapping/disappeared b/c feels like there'd be lots of easier ways to get rid of a child /change their identity/send them off overseas like the others as opposed to straight up killing them (my theory for that is that it's some mistress or politicians illegitimate child that would've caused a problem in the future if he grew up and publicly known/connected to the father or so)

I don't think the twist was planned, we saw too many times that Ao was new to this and inexperienced even when no one was watching. I'm sorry author but there's no way this wasn't a spur of the moment decision.
