Not quite sure how to fully interpret this, but here is my understanding:
The kid actually went to an abandoned hospital, where there is some severed head lying around.
He said he visited with Miyo, but on the 1st and 4th panel, you can noticed that Miyo does not have legs, and he had drawn legs for himself on 1st panel, so it could not be a mistake.
I think he has been seeing a ghost of Miyo, and Miyo is actually murdered in the hospital. Her head might be one of the severed head the kid saw.

He also mentioned "This is where her head was cut off mistake.". The past tense "was" might suggest that he was at the scene when Miyo was killed. No idea if someone have killed Miyo by accident, or the kid himself accidentally killed her.

I actually interpret it as the teachers being correct about it being a hairdressers and mannequin heads, but missing the fact that his friend (now a ghost), was previously killed at the hairdressers.
I think the clue that this is correct is in the fact you see the girl getting a haircut (and more) on the last page. This series tends to be fairly on the nose in these things.

But either interpretation is fine. Friend Dead, they lost their Head - is really the whole story; Where is not important.

Miyo is a ghost, She told Ryu that her head was cut off there (by mistake?) And so that's why Ryu goes there with Miyo and found other heads that was cut off (by mistake).

The place was probably abandoned hospital where some malpractice happened, or maybe dark magic experiment idk.

Before hint: Probably was playing with a ghost girl and she lead him to the place she died. Since there was more than one severed head, I'm assuming it wasn't an accident at the hospital, but a serial killer dumping the remains of his victims there.

After hint: She has no legs, so yep, a ghost. Only question is if she was murdered though.

huh, for a second I thought her legs were taken off despite the focus on decapitations, but considering how Japanese ghosts are depicted without feet or legs, I think you've got it.
