Cursory wiki check indicates the urinary tract is lined with smooth muscle like the stuff that lines your respiratory and digestive tracts; urination happens in part by that smooth muscle relaxing. I didn't do a deep dive into research on incontinence, but "having half as much urethra as before makes it not as easy to hold it in" makes some intuitive sense with that information.
