I'm pretty sure Aqua has no romantic interest in Ruby. She's crushing on the doctor she knew in her last life, not Aqua as he is now. He's technically the same person, but he's changed a lot from his last life.

I think meanwhile, he sees her as both Ruby and Sarina. And he never had any romantic attachment to Sarina, he had more platonic and familial feelings towards her. So the crush feels one-sided.
