Might not be as bad as you might think. Here on earth we need to be careful about genetics due to how incestuous our ancestors were and the utter lack of the ability to easily fix it. Fantasy magic and orr having real gods could hand wave those genetic issues away.I ... don't know what to think anymore.
All I can say is if the siscon prince really wants to stay 'connected' to his sister in some way, without putting much risk in their children's health, he should play it safe and have their grandchildren marry each other. 2nd cousins within the current royal bloodline should be just distant enough from each other that genetic disorders have the lowest chance of developing at the earliest time possible.
So in fantasy settings there might not necessarily be reason for there to be a taboo against sibling marriages. That's not to say we should go down the 'I am my own grandpa' route, but the occasional sibling marriage wouldn't be as harmful as it would be on earth. It would mostly depend on how much genetic damage there is on the family tree.
