After about a half-year bought of mononucleosis when I was 16, I was left borderline anemic, and that didn't have me coughing up blood. It would be either a more serious disease or injury, and there's no damage to her clothing or armor....

Looks like standard fantasy gear to me. You put on the biggest, fanciest and spikiest bits of armor on legs, hands and shoulders, and leave the face and most of the torso bare. It's supposed to look heroic and sexy, not necessarily practical.
And honestly, anything sturdier than these armored pyjamas could be hard for her to lift and difficult to breathe in.
