The girl had not yet graduated at that time. It must be remembered that the age of consent in Japan is 16 years old, that is, she would be at least in the second year of high school, so there is no outrageous reaction for the teacher to have a relationship with the student, as it is not pedophilia in Japan at that age. It appears they started a relationship while she was in school, and broke up after she graduated.
Note that the author is purposely leaving it a secret as to whether she died or not, giving evidence that points to an ''attempt'', but unfortunately there is little information yet.

But that still doesn't make sense because if classes were going on, why was she going to see him in his office and asking him about a lesson they did earlier? Also why was the male MC just randomly strolling about the halls looking like he had nothing to do?

The way it's set up she apparently had a meeting scheduled with him (she mentions being "late" and he excuses her because of student council work) which isn't something you would do during class hours considering she would...you know...be in class.
