The grandson's job is to go out and find potential female victims and get them to the island. Kusaba is an outsider that discovered the truth and had to escape into the wilds because the villagers want him dead.

There could be some kind of serial killer version of Godai. Someone who's body hopping around in the past committing murders.I kinda expect this to be another case like the nurse, where the publically known killer isn't actually the culprit at all.
He might just have been the only person alive at the end, maybe even gone mad.
If the "meatball" really looked the same in both cases - the island, and MC family - that would imply the same killer. Two Killers doing the same weird thing is a bit unlikely.
