Never been impaled with something like that, but I can only assume you're in so much shock from the original injury that the new pain isn't even noticeable. Tissue damage would be way worse from the fall if the foreign object twisted/bent/sheared though. Tearing tissue always does more damage than piercing, which generally moves the tissue out of the way.

Yeah, it's a bit funny here because you can see how far it's sticking out in front of her and behind her, and yet when she falls down she falls perfectly on her back with it not being pushed out at all from her chest. Either it would've broken on her fall (which it doesn't look like there's an indication of), or she would've been forced onto her side, or the force of falling on it would have pushed it through her chest and it'd probably fall out + make her blood loss a lot more severe.

The way it is now only really makes sense if it broke + we weren't shown, or it perfectly slotted into a hole on the floor.
