In chapter 16, it was confirmed that other people don't see the Loli Spirit. In chapter 11, Myaako mentions that other people have seen her in that form.
So probably Option 2. Strangers don't know she's a bakeneko, so they'll think she's a loli wearing Nekomimi.

P.S. Loli Ghost, scared of ghosts. Bakeneko also scared of ghosts.

P.P.S. Loli human got sharper fangs than Loli neko.
