Probably, he's putting a similar if not stronger mental burden on himself towards his new sister because of his original one. His feelings towards his first sister were twisted enough as is and seem to have ruined his romantic feelings.

Since new sister is his sister but also a game character he knows as a separate existence I expect lots of obvious sexual tension and him saying how he wants to marry her off and him thinking about marrying someone but then Yosuga no Sora ending.

I believe it could be a mistake OR the adaptation was covering the original web novel where his character had died and then the mangaka was told by the editor that the light novel, that is the official novel, had kept his character.
For MC referring to the sibling as "he" when his thoughts about the future clearly show a female uniform and say is a heroine, I think is a bad attempt from the mangaka to keep the original surprise of she being a reverse trap that faked being a boy for noble family circumstances, MC knows she is a girl.

Went and reread the story but I didn't see the portion where he said that. From what I understand anyways, its most likely that its that he's alive but he's simply a minor charactor, basically one that only shows up in flashbacks but doesn't appear in a major scene. He isn't so mistreated to the point that he dies but he doesn't really have a standing in the family either since no power
