then since his parents are unfit to be his guardians, he gets adopted by relatives from Tokyo (Tamayo)

Isnt that who I was referring to? Shiratori Hikaru is the guy who was studying to be a doctor with her right? The idols brother, pretty sure last chapter of the prequel, he says to her "Do you want to marry me then?"
In any case I hope those two did get married.
