See my followup post, it was already there on the panel where the portal first appeared, before he got stabbed (unless you want to say Demonio was off-panel already having stabbed him at that point, but I doubt it). He also spit blood earlier in the chapter because Demonio punched him in the mouth last chapter, so we know his mouth was already bleeding.
