I think there is still the possibility that Kiruko's brain is still of the older sister. It could be a strong act of self-conviction (as in another famous manga that I don't want to spoil

) perhaps due to mourning or guilt for the loss of the little brother. Even stopping menstruation can be caused by this

I honestly hate mari- excellent manga, but I hate it so much- I'd really rather that not be the case

The manga has serious issues with tonal whiplashes, but it does foreshadowing really well just like the anime.
