Mimihime saw him as a hiruko while speaking to him while they were in the medical ward; she said she felt she could see the future and tried really hard to stop seeing him as that slug.

It seems that Maru’s ability only work on the genes of the children from the academy, that’s why it works on Totori back then too.

One thing to notice too is that he can sense Hirukos from a distance when they have turned into monsters, but not when they're still human. He could not sense Totori was an Hiruko, nor the two radio broadcasters, nor all the others he met.

No Maru developed the ability to sense both human-Hiruko and Man-Eater Hiruko by the time he met Jūgo in chapter 29.

And Maru stated he has sensed the many Hiruko-people in Nara

Let's describe here a small theory I made just for the fun but after posting in on the Reddit subforum, some expanded on it.
In chapter 35, there is a dual panel showing all the kids in the school. Among them, you have this one :

Same hair color, same eyebrows as someone we know well.
It's one of the last generation kids, 5 years old when they are introduced, soon before the disaster. If he or she was still alive at the time of Maru and Kiruko's journey, she (let's assume this character is a girl) would be around 20... Like Kiruko.
(EDIT: Actually I'm wrong here, she's the generation before so she might be one or two years older, but Kiruko's age is quite blurred so I don't think it invalidates the rest of the admittedly crazy theory).
In chapter 38, Aoshima asks Shiro and Mimihime where are "the three others", and Shiro answers only Anzu and Taka left the school with them. So another kid might have disappeared, we just don't know who. And I could not find this character appearing again in any drawing after the one above.
Could it be that this kid escaped alone, was found and adopted, named Kiriko and dyed her to have the same color as a younger brother? Hair color that would come back after a brain swap because she does not dye them anymore?
One thing interesting and linked to your above remark and to your hypothesis that Miruka would not have turned into a monster because her brain was transplanted, is that it would explain why Maru do not detect Kiruko as a Hiruko: as you imagined, the Hiruko core must be in the brain and is removed in the brainswap. And without it, no detection from Maru.

I doubt it, since we saw Kiriko and Haruki as children in 2024 already in chapter 44. And Kiriko doesn't look anything like that.

You might be right but to be honest, the school kid looks a lot more like adult Kiriko or Kiruko than the above drawing, so I'm not sure you can draw a lot of conclusions from it.
This theory is probably a bit too wild, I guess, but there are many hints that fits well with it: for instance, Kiruko's name is very close to Hiruko. Granted, it's a contraction of Kiriko and Haruko, but it's interesting that in chapter 35, after being called "Hiruko", one of the child says that he had the impression that he was called by his real name. And others confirmed to have the same feeling. If the girl escaped on her own and was found but people outside, when asked for her name, she might have answered "Hiruko", which may have been misunderstood for "Kiriko". And later, when choosing a name after the brain transplant, Kiruko would also fit unconsciously.
I could not find any other drawing of this character, which would indicate that she might prefered to stay alone and would explain she would escape alone from the school.

instead of them being literally the same person, remember that all the kids are clones of existing people, so perhaps Kirukos red hair etc somehow comes from some the same original donor... of course this makes just as little sense. I'm personally suspicious of who Haruki/Kiriko's parents were (Takehaya / Takahara, perhaps they have some hiruko in them as experimentally grown children too and this were given such similar names) but this is all pure speculation. Appearing in the cave however is a big red flag for being some kind of unusual.

Moreover, since it is suggested that an Hiruko escaped at the same time as Taka and Anzu got out of the school and reached the sea, it's a lot simpler that she escaped (or was moved out) of the school before the asteroid fell, was adopted and became Hiruko's sister after that. We can imagine that her parents would dye her hair black either because they wanted the two children to feel connected or simply because one of them helped her to escape and wanted to hide her. And that's why at the moment of the disaster, we would see her cry with Hiruko with black hair, probably because their parents died.

I actually considered the hair dye theory before too ahaha, which would be pretty interesting, but the problem is trying to reliably find hair dye in the post-apocalypse. Maybe that's what the little "finding paint" joke was during immortal order (which Kiriko also appeared as a cover art)

Well, for any other manga, I would have dismissed that as just us projecting all kind of stuff on it. :-D
But with this one, there are so many details that are not left to chance that you could totally be right.
I like the idea. Good spotting.
In fact, it's interesting that you point this moment out. Since Kiruko sees Hiruko and Kiriko in the two kids going away, we can assume that their respective attitude reminds her of her old selves. That would mean Hiruko would be quite friendly to others while Kiriko was probably wary of strangers. Which would be consistent with the character of school kid which I assume is Kiriko, since we don't see her befriending any other hiruko, nobody seems concerned she left, she probably left the school by herself, so she was probably quite lonesome. And Kiriko is never shown as having friends, contrary to Hiruko. In fact, the only person we see her with is the doctor.
When Maru says that he might loved Kiriko if he had met her, that's probably true since she would probably have been distant, while Kiruko is as friendly as Hiruko was.

One thing interesting is that at the end of chapter 17, the author tells us what is easy to find and what's not. Dye does not appear but soaps and towels do appear as relatively easy to find, while medecines, batteries and alcohols are difficult. So maybe hair dye is not so complicated to get than we might think, since goods related to bathrooms seem still available.

Welp. I binged this straight after seeing episode 8 and now I'm waiting on a monthly ongoing manga! Well done, idiot! But on that note, I'm really surprised and worried this got such a great adaptation already. There's a lot of content to adapt but not that much, and new one will only trickle and never fast enough. I fear that long delays will hurt the reception of the anime at some point, when the story really shines through that medium.

Anyway, I'm mad that the vice-director simply abandonned Maru at some point? What the hell did they need him for, if it's to leave him alone in an orphanage. And it's very strange that the Director in Nara's body found him there and sent him on this mission to inject Yamato with something. At least, I'm glad Sawatari gave them a lot of answers already because Kiruko really didn't press him that much about the whole brain-swap ordeal.
