Hmmm, if he became a hermaphrodite, then they both share both sex organs unless they have unique ones on their individual necks instead of their body.

But whether the organs are shared on the body or separated on the neck, I guess the other head identifies as female based on him identifying her as female. He would know best.

Side note: Sex organs on the neck would be quite unexpected and is something I've never thought of until this post.

Can't say anything about the accuracy of translation, but IMHO the term 'hermaphrodite' is a bit misleading. With his twin heads, both sexes and command over death and life it's kinda obvious the 'Ouroboros' in this world represents duality, and since this is Asian story, it's most likely inspired or influenced by the Yin/Yang philosophy. So actually, he is less hermaphrodite, but more both male and female and as such the other head being female makes sense.

He's far beyond evolving into a human at this point. Even if he could, it would be an insane downgrade. Humans just aren't that strong individually, and they also rely on gear, weapon-skills, and magic, none of which he is good at.

The LN has Irushia note that being a hermaphrodite must refer to his heads being male and female because his nether region has not changed like that.

Most likely:
When you want to say 'I' in Japanese, you can use 'ore' (aggressive/vulgar masculine), 'watashi' (very polite and more or less feminine) or 'boku' (informal masculine). So if she refers to herself as 'boku' (like a tomboy), it can cause a confusion.

Was that the official light novel translation? I will use that then. but penultimates tl description was this + katakana closely matches with it:

yes you are right! Shoujo (少女) and Shoujou (猩々).
