That just looks like the view from the bottom of the cave they're in. Unless there's kanji hidden in the scribbles, I think it's just the author's mind cracking from the situation.

I reread the first two chapters per the author's hint and wow it's so fun noticing everything on the second pass. this is a really nice pick spira, it starts out misleading you into thinking you're reading a very linear lost-in-the-mountains supernatural slasher, but it's actually closer to an agatha christie murder mystery that rewards you for combing over earlier clues like some of your other scanlations

not sure about anything bcs it hasn't been confirmed, but for starters:
  • ch 2 page 13 seems to be ambiguously talking about the goblins ("must've been those from the town below (...) they use this pilgrimage path")
  • ch 2 is also very informative, e.g showcasing children's belongings being piled up, the goblin only tries to abduct Hajime (the short one) with no interest in Umi. (also note that Umi's little brother was abducted too) - i.e, they only seem interested in gathering children. whereas women seem to be entrapped in cysts to be liquified/digested
  • we see the old man at the shrine be very explicit with talking about the cave like an animate entity ("the cave is hungry, it wants to be fed"), and the way that there's a sudden geological rumbling making them fall into a hole they peered into? I'm inclined to take this all at face value. at the moment I'm assuming these creatures are simply worshippers of a larger biological system
  • so, we have women & children as targets, we next get a peek at what might really be going on in chapter 14. none of these disappearances are on the news, rescuers are prohibited from entering by "the mountain's caretaker", Riku's wife says it's happened before & she'll investigate, then a shadowy figure appears in her home (presumably to silence her or whatever).
  • this to me makes me think the author is hinting at the idea of partial collusion with authorities, the thing we don't know yet is how anyone stands to benefit from this arrangement
I have an unsupported guess based on vibes right now, so I hesitate to call these clues:

  • revisiting ch 2, isn't that an oddly sentimental way to be carrying around a dismembered head?
  • when akane is unearthed from that mucus-like cyst, I couldn't help but think "egg". isn't it strange how she talked like normal prior to melting? if it were just a hallucination or deception, why would it have her memories?
  • we also see these tree-like strands of mucus emerging from their noses, and it's almost always prior to that person being affected by a hallucination of a loved one
  • and then there's the white roots of the lake in the pit that they're thrown into

so, the question is: if these are roots, what form of sustenance are they looking for?

oh wait my wild guess (I'm putting, like, 20% odds on this one) is that our Hajime isn't "real" in some sense. that's why his dog barked at him, why his flashbacks only ever lightly dip into Tokyo, the weird timing of the party welcome back message, him still looking identical to his graduation photo, hinted at Umi saying "here, you can just be the old you" in ch2.

wilder guess is Umi (the older brother figure with dyed hair) organized all of this deliberately and knows more than he's letting on. motivation? idk maybe as he said, he really does think that "it's situations like these that bring people closer together" (deranged camp counselor), or he's still trying to find his brother. his cavalier personality would make that kind of reveal a nice rug-pull on audience expectations. it could also be that Umi is the one who isn't real, but that's more tenuous since there are only 4 hints for that.

btw I'd really recommend just rereading it again, it's simply more enjoyable picking up on little details the second time around
