He's in an unknown land and he's so generous and unquestioning of everything. He just accepted everything that happened to him.
His personality is another factor in this equation. He's rather understanding of his situation. If he weren't to accept it, it could cost him. I assume this part of his personality was essential for being a doctor.

I know this is par of the course for isekai stuff but, going back to the first chapter its so fucked up that this island has been wtihout a doctor they finally get someone to come over and he gets fucking ZAPPED IMMEDIATELLY. Imagine them having to turn around and tell them "hey can you send someone else over? he is already fucking DEAD". Will take them a long time to get another one if ever at all

Well I've got good news for you then, this first chapter I think might even be a parody of Dr. Koto Shinryoujo, which is about a doctor getting relocated to an island and NOT getting isekaid.

Don't know if it's the raws or the translation, but I thought cirhossis is scarring of the liver from accumultion of damage. The varicose veins of the esophagus is a different condition that can be a consequence of restricted blood flow in a scarred liver. The varicose veins can rupture and show up as vomiting of blood.

MC seems to have experience in multiple departments, doing endoscopic procedures, seeing children with musculoskeletal or genetic conditions.

I love this manga, but rereading the nurse conversation makes no sense. An accomplished doctor going to a remote island for work isn't a one way ticket, he'd have hospitals BEGGING him to come over if he felt like he was done there.

Honestly i think if he stays on the island to be the village doctor would also be an interesting story. Also I kinda feel bad for the village. They waited for years to finally get a doctor just for him to get isekai-ed. Hope they get another doctor.

It's probably true that it woundn't be a 1 one ticket for an accomplished doctor. But from what I've seen, he probably wouldn't agree to come back if the village does't get a proper replacement doctor. And he's never gonna be "done there". I mean people don't stop getting sick so a doctor's work is never done.

Also idk what the rules / laws / ethics are in Japan but I assume the nurses said that because he would be the only doctor for an entire isolated comunity, so people probably wouldn't recall him unless they have someone to replace him. They wouldn't want to leave the island w/o a doctor. And a job on a remote island is a guaranteed no career advancement, plus having to be away from everyone you know. I'm sure there wouldn't be many people want to take that position.
