This is a twitter manga though? It's not as far as I know subject to the Tokyo law which restricts displays of incest in published serialised series sold in tokyo to not be shown in a positive light in media if sold to under 18s.
The implications being that there's no reason for them not to be actual siblings unless the series gets serialised commercially and the author has to sanitise it.
Look at Haruka Hano's currently serialised sister and brother series published in Dengeki Daioh, Lots of drama and a depressive atmosphere but the siblings are real siblings.

You even have some version of Oreimo trying to be like "they step-siblings" despite them obviously being siblings... (I believe it was the videogame that did that).

I believe that was just a "what if" scenario that you could see after you beat the game, so no cop-out there, simply showing that, if it wasn't for the moral problems, they would've been banging THAT VERY SAME NIGHT.
