This isn't medieval by far. More like late 19th century. They have automatic guns and everything. I don't think we have seen steam engines yet, but I imagine they might be there. Clothes, architecture, interior design, and many other things are far from medieval, but those are more ambiguous things in manga in general. All in all, this isn't super consistent. One particular detail the author has gone to great lengths to avoid depicting is lighting. We see an occasional candle around, but obviously that can't be everything, plus there's no way those royals would only be using candles anyway. I feel like the author simply has little interest in the environment and technology considerations. Fair enough, seeing how the story and especially characters are good.
