They used horse carriages and swords, with occasional firearms. they had antiseptics and anesthesia, IV bottles, and surgery, but other aspects of medicine were rather primitive. Things like that might have worked around the turn of the 20th century, where the disparity between the electrified cities and the non-electric countryside existed, but the setting was in the imperial capital.
