MC doesn't have cultural context and Japanese get confused by how italians and some other europeans actually do use kissing as a greeting. It's the sort of thing a Japanese person could easily get tricked into thinking far easier than a person without exposure to european cultures.
In short, MC only looks dense to us because we realize that Italians don't use tongue on their sister.
