Testicle cancer is an oddly specific wish to grant, specially if it's the only one she can do. Wait, was it supposed to be a joke? But in-universe or for the reader?!

Maybe reality warping needs some visualisation and she can only imagine testicular cancer, it could be used as a weapon just grand cancer to criminals, political/romantic Rivals or assholes they never find the assassin who killed some by wishing cancer.

I think it was implied that the wish is for yourself, thus making it impossible to wish it to someone else. That would be a whole different wish.
