Wonder if that means next chapter the truth comes out? Feels entirely wrong, pacing-wise, but.

That or he crashes out and runs away and we get a bit of self-pity where he avoids her out of shame and self-doubt standing next to the Extrovert, but I can't see that happening mid-festival arc, either, when the fireworks haven't even happened yet.

Each arc usually ends with things going "back to normal" between them, but MC's been getting more and more in his own head since the fourth member has shown up, and so I kinda suspect we'll hit an actual point of jealousy and misunderstanding before too long, except he won't know why he feels that way since the MC hasn't clued in that he likes her, yet.

Festivals are always when feelings are realized though in this genre of manga, so I'm betting this will be the start of MC realizing he likes the girl whose face he can't see, and everything beyond the point of this Festival will be colored through that lens, while he still freaks out over not knowing what she looks like, and the fact that he can't act "normal" around her, and she either knowingly or unknowingly carries on in her usual way as he gets jealous and self-conscious and anxious.

prosopagnosia, unless congenital (everything I've found says that "congenital" is the least common occurrence), is usually acquired due to severe brain injuries - which would make me think this would instead need a 'medical' tag.

While true, I don't think prosopagnosia tends to work like this. That said, that's never stopped an author before, so it would be at least a little interesting of a reason.

I mean yeah, unique premise and all but dude is missing a VERY important part of interpersonal relationships. The fucking face. He can't know if she is sad or happy or fucking depressed unless explicitly told. There is only so far you can go without addressing this problem.

it's just the closest I could get in terms of something "not-supernatural" that this might be with what I know of the medical world.

I also think it'd be fine if Wakao were actually a yokai who liked Mizuno and so he was the only one subjected to this facet of her, though, for reasons we just haven't heard yet.

But he is only not able to see her face in particular, he can see others just fine, while everyone else can see her fine so it is supernatural.

It doesn't seem she even knows he can't see her face, I think it's a condition related only to MC

its clear she likes the MC right. it might be something like her not wanting to show him a fake face, or just her wanting to see if he would like her even though she isnt human

I think it could be even simpler - not so much Wakao not wanting to show him a "fake face", but just that she's shy/bashful around him in general, but can deal with it if she hides her face from Mizuno.
And when she "breaks composure" and gets all blushy once per chapter, she's always turned away from Mizuno, or behind him and out of his line of sight.

Which makes me think that her face is revealed when she loses concentration because of "doki doki" moments from Mizuno, so she hides away from him.

If I'm correct in my read on that, then I think she's aware he can't see her face. Especially if it's because she is in fact a yokai, and I have to assume that she'd know how her own powers worked, whether they were consciously or automatically in effect.
