Could it be Ibaraki Douji? With Kasen helping Reimu, and Ibaraki Arm somehow having gotten loose and working with Mizuchi? They technically count as one person, and the latter could have been freed by the same person who initally caused Mizuchi to get loose.

I love the Touhou manga for small details like that. Satori's the only one who's out of breath after climbing the stairs to the shrine. Nerdy bookworm gets 0 exercise.
