It's a two fold issue from my PoV. Ones that you can find in most cultures the world over.

First is the stigmatized insult of having a bug associated with filth invading your home. True not as harrowing as more manga try to paint it, but this media is nothing if not exaggerating in their portrayal of scenes and events.

The second is the typical reaction to small vermin that skitter or filt about. Especially so if they drop from above or their chaotic flight patterns have them randomly crash land on your face. Again the whole filth association comes into play, but in a more direct "it touched me" sort of way.

It takes a very strong will, or truly desensitized person to ignore the creepy crawlies that invade our personal spaces. Mosquitoes, spiders, roaches, moths, and many others have created a special trauma in the human psyche. Oh sure you act strong and tough now, but the sound of a mosquito buzzing in your ear or a roach skittering accrossed you bare foot is going to send you flailing and stomping about. Might even pull a squee of terror from your throat.

Think of it in the same way as spiders and people who suffer from arachnophobia. Many in Japan suffer a similar phobia that it's to the point they don't even print or say the word cockroach. They just refer to it as or with a G.

It's exaggerated for comedic effect. The censorship is also a common joke, in the style of "this is so terribly grotesque we cannot show it"

They can FUCKING FLY. Have you ever even encountered these demonic beasts. I know I've only seen them during family visits to Asia, and they are many times worse than the common bugs back home in the US.

Thats the thing. Cockroaches are common in Asian countries. You never see bugs of that size invading your house in NE states. The sheer volume whittles down the general public's capacity to face the demon, so more people who (rightfully) fear it are forced in situations where they must deal with it somehow, enough that it becomes cultural mainstay.
