I've come up with my own theory as to this.

Since we know that some weird supernatural power is in play in this whole "absolutely will come true spell", it could not be wrong that some type of paradox is in play due to the overlapping spells of Usagi's and Tora's on eachother.

As for Usagi's death, it may have to deal with the last part of Tora's spell "may Usagi not belong to anyone," as every time Usagi tried to confess to Tora, she would be killed.

And I assume they didn't loop again at the end, because Usagi didn't do the spell in the last loop.

Honestly I was confused a bit too. I had to re-read the last few pages a second time. I think essentially Tora actually gave Usagi the hair to make her wish to be with the guy. But at the same time Tora made a wish that if Usagi wouldn't be with her then she couldn't be with anyone. And the person with the more powerful feelings wish would cancel the others out. And so the wish decided it would just end Usagi instead. (Careful what you wish for)

Strike-through Edit: Nvm Tora gave Usagi her own hair like I originally thought. So idk it's a bit confusing. I don't see why that would cause the accidents but whatever I guess
