I know that. I understand their reasonings. I just asked why do a lot of manga protagonists as dense as this? Especially when most of them doesn't have such reasons for their dense-ness.
In his defense: It's the fucking zombie apocalypse. As long as you don't currently have a girl/boyfriend or a crush on someone, love is probably the last thing you're going to think of. And if he only used her in his eyes, why would he think she likes him or thinks good of him? Especially since she as her brother(s) to worry about.

Like I said, I was talking about OTHER dense protagonists. Not the MC of this manga because his "dense-ness" has proper reasons.

I kinda find it funny that the poor childhood friend was trying to be magnanimous, but the girl didn't even think about him at all. And her half-hearted "sorry" was more like apology for dumping him, but he didn't realize it.
Just look how hard she needed to tell him that she loves him.
And I can't even sure that he actually understood.

This is not even the MC being dense. This guy is an actual misanthrope with very little empathy for other people. He only see relationships in transactional terms. Note that he has never thought about his friends or family. Probably didn't have any in the first place. Likely that he never loves anyone either.

Tbh it's more of suspension and logical choice. Remember he's the one who protected her and her siblings. He did a lot beyond what a simple sex for food was supposed to be.
You can contrast it with the childhood friend who only tries to help her when it's easy and convenient but switches when it's not.

It's supposed to be a self insert template for typical Japanese males. One it makes it easier to have the protagonist be passive and indecisive, opening up multiple heroines and avoiding hate from stupid actions the protagonist may take(read author writing a terrible rizz) that will garner hate.
Second is as someone stated, in today's dating world it's hard for majority of males to pick up clues and understand when a girl is romantically interested in them. So they end up assuming it isn't ( there's also the Japanese ideal of polite gentlemen and asking that kinda stuff to someone uninterested is rude af)

In his defense, everything "was" misunderstanding.

he simply asked for sex favor when he's snapped. He just want some information while he was unconscious and nope out after got all informaton from her but mitsuki was deperate enough to accepted it. so he changed his plan and move his base from his own apartment with 2nd best girl to shopping mall. that make him indirectly helped her more than enough since it's already his base.

He afraid that he might be virus carrier and his fluid might infected her. so he stayed by her side while she was sick. just to make sure if his theory was right or wrong. she thought he was "that" worried about her.

then they bonded during sheltering in the shopping mall for quite sometime, to the point that he blamed himself for not giving her a gun, make everything fall aparts during creepy guy attacked. (which he didn't notice it himself that he already trusted her to consider giving her a gun at that point)

after the "creepy guy" incident, their relationship is already at stage of "almost progress to next step) if they got time to sort it out but Ak-kun(blondie childhood friend) intervened before they got that chance. so they seperated with more "misunderstanding" (silly troupe, I know)

and in her defense, She feels like never truly give him anything in return except from sex favors. so she feel inferior to doctor hence dragging out the plot more.

The others manga are usually inferior complex trait MC in overpowered isekai/romcom plot where normal MC somehow having unique talent that won't be use anywhere except cover FMC weakness.

Bro, thank you for the long explanation but I understood MC's reasonings in this manga.

In my comment, I asked why there are a lot of manga protagonists that acted as "dense" as the MC in this manga, despite not having any appropriate reason (as this MC had) of why they become so dense. Most of them didn't really have that bad of an inferiority complex, and yet they thought that they're unloved even though females left and right were throwing their body onto him. It happened too often in harem manga. What a weird culture.

Not much to add to other people's comments. Just wanted to say, read the wiki on Stockholme Syndrome. It's not in the DSM-5 for a reason. Historically, it's less they fell in love with their captors, and more ACAB. The hostages, saw the cops being assholes and doing more to get them killed, than the actions of their captors.

The Suspension bridge stuff is weird, but at least has more actual science experiments behind it. https://en.wikipedia.org/wiki/Misattribution_of_arousal Humans are messy, in all senses of the phrase. This doesn't stop the media, the public, and authors from confusing the popular view, versus the actual clinical views of what happens.

for what it's worth, you could try to justify the mechanics of the relationship using psychological models or anecdotal cases you've seen in real-life,
or you could just accept that this is how the mangaka wrote it because he controls the story
i don't think he thought too deeply about this, and how it works out logically.
