I was wondering where was the original star maiden since they have the same fate as the main character. But I’m glad she didn’t witness everything that happened.

I think it's fair to say it was at the condemnation, when she 'passed out'. Go back to chapter one and review the description of the changeover from Emi back to Remilia.

You can interpret it as Emi's inner dialog up until it says "My whole body grew still and that was when 'my' consciousness was cut off." Then her next inner dialog being this chapter, (which starts with a review of her experience at the condemnation) where she is saying "I'm sorry, Remilia-tan" while waking up.

Although I don't know anything about the LN, I would have to say, the manga did better than the WN with the whole Emi-Remilia interaction and Emi rebirth. This was wonderful.
