Wait. Im thinking here. So yuuki and mao were childhood friends. So they were friends back when Yuuki was a girl?

But they seem distant. Did they date when Yuuki became a guy and things ended poorly.

Is that why things seem awkward when those 2 are together.
Seems like Mao didn't know Yuuki had the disease either, and she put 2 and 2 together that it was Yuuki right then and there in the conversation.

So, the "gender bender high school comedy" period is over and we're back to "its a code word for trans" style? Well, I don't want to pretend I understand how trans people feel, but I feel like the conversations the MC had with her friends in this chapter were incredibly painful, like the real experience of discussing your trans experience with your close friends for the first time.

What do you mean? I see shitheads like him in the comments all the time.

They are very much real, and far worse than the toned down version this story shows.
I think everyone had that one friend or acquaintance who constantly made their life uncomfortable by discussing someone else's sexuality, gender, or personal life in general. For example, my childhood best friend thought it was funny to spread rumors that I was gay because he expected people to easily believe it considering I was very private about my personal life. Even though he knew perfectly well that I was interested in girls even more than him.

I mean, obviously I wouldn't want to interact with such a person in real life, but as a character in the manga, he piqued my interest, to the point where I am interested in his story the most.

I'm just saying that the mess he creates affects people on a pretty intimate level, so at least in my personal eyes it's a bit beyond the usual (unintentional) cringe humor.

Not the feeling I got. Yuuki and Mao are just making up a fourth person to not expose to Mino the fact that it happened to Yuuki. Hence the boldening of the word "fourth". I think it's pretty clear Mao was fully aware of what happened to Yuuki, specially since this chaper also basically tells us Mao's twin brother also knows, but it's unclear if she knows Yuuki has/had a crush on her.

IMO Yuuki and Mino are meant to parallel each other.
Both were born girls and got the disease that switched their sex, but seems like Yuuki is much more comfortable in a male body than Mino, partly because he always loved Mao and saw her sexually, even before the switch. He easily switches to thinking of Mino as a man even if not on purpose, and is conflicted about her being around Mao. His inner conflict is much more centered around what other people would think of him if they knew, and being afraid that Mao will see him as disgusting for originally being a girl.

Meanwhile Mino is basically the complete opposite. She's aroused by girls because of the rising hormones (which the doctor explained is a normal symptom of the gender switch early stages), but she's still romantically attracted to men and thinks of herself as a girl. She's worried about her crush being impossible because of the way her body is now, wishing the switch hadn't happened, and wants to earnestly return to how things were before. Her fear for the treatments proposed have to do with the consequences and the fact it's not gonna be a "back to normal" solution, not any attachment to the current male body.
Her biggest source of anxiety is also of people learning the switch, but unlike Yuuki's "afraid of people thinking he's gross for having been a girl", hers is "afraid of people thinking she's gross for currently being a man".

I'm pretty sure there'll be a lot of future conflict eventually over Yuuki thinking "Mino's similar to me" only to discover she basically got the opposite experience. His disease solved his gender dysphoria, while Mino's created it as she was a cis straight girl.

Yuuki is a was a girl who liked girls and became a guy which she hates.
Mao was probably the girl he liked.
Mino was a girl who liked Yuuki but then became a guy too.
So would this be yuri or yaoi.

There's no way Mao wouldnt know Yuuki got the disease if they're childhood friends. How would one not notice the girl they hanged out with throughout their childhood suddenly became a man.

As of this chapter, I think everything still lines up with it quite well. You might want to reread ch. 12 & 13, I think those two give the most hints towards the character's past.
