So people thinking the dad's stroke was kind of pointless, guess here is the reason for it narratively.

It gives Kaoru doubt about the future, feeling the need to move and continue to live with his dad even after being an adult to better take care of him, but also worry for not being there for Nanami but then also a reason to Nanami a reason to showcase that she'll be fine even if Kaoru can't be there with her daily for awhile, she's not how she was before and she will be okay(even if the obvious end goal would be the three just live together but that can be a future thing).

If this were real life, this moment right here is when she'd move on from him. Most relationships don't survive long distance, specially over a few years. Hell, most relationships don't survive near-distance.

Honestly, I'd dig if they each went their separate ways, and then have a friendly, non-romantic meeting years later in an epilogue. It doesn't happen often enough in manga/anime. Only one I remember doing something similar is Byousoku 5 Centimeter.
