This is kind of the culmination of Nanami's character arc; the ultimate test of her growth as a person--she is deliberately discarding her safety blanket to be an independent person. Sure, she WANTS to be with Kaoru, but she's trying to demonstrate she doesn't NEED him, that she is a person separate from him.So people thinking the dad's stroke was kind of pointless, guess here is the reason for it narratively.
It gives Kaoru doubt about the future, feeling the need to move and continue to live with his dad even after being an adult to better take care of him, but also worry for not being there for Nanami but then also a reason to Nanami a reason to showcase that she'll be fine even if Kaoru can't be there with her daily for awhile, she's not how she was before and she will be okay(even if the obvious end goal would be the three just live together but that can be a future thing).
