I think in typical Yuri stuff that tries to involve kids, it's along the lines of adoption (less legal form, more just taking in a kid and treating it like a family), pseudo child (think some kind of cutesy spirit created from both their mana or lab experiment in the rough shape of a kid or something) and it's at the most extreme end we get magic/science babies (mcguffin to allow an actual pregnancy with both women as a parent). You gotta understand that in a lot of Yuri works, even the presence of a man in any shape isn't really depicted beyond background fodder.

It's likely a difference between "The Power of Plot!" As the OG Hiro is MEANT to be a scumbag villain who DARES to get in the way of Yuri and is meant to be a Hate Sink. So with the POWER OF PLOT, he gets to ignore the LOW SCORE disadvantages as he can't be EVIL and "in the way so he can get RIGHTEOUS JUSTICE" if it DID apply. But since MC Hiro has to LIVE in the world before the main OG story begins but LONG AFTER OG Hiro has "established his rotten reputation" he is BOUND by those rules (A Low Score for example was likely there to REMIND the audience OG Hiro is rotten to the core, but is ONLY brought up when Plot needs to happen or something like that. But for MC Hiro, it's a PLAGUE because it means HE HIMSELF is limited) and sucks for MC Hiro, since the "Bad Guy Horns" (Opposite of the Protagonist Halo) are fused to his head. Bad things have to happen to him so the world reminds "he's the bad guy" (It's lie Wacky Races where at the time of the show, there was a RULE that "Bad Guys can never win" so when the Villain of the Show won a race LEGIT, as in not using his USUAL cheating methods or such, he was screwed out of a price like the other racers or even disqualified with some bullshit rule! Yes kind of Bullshit since it's saying "Once a bad guy, ALWAYS a bad guy" meaning even IF they do good, "well their the bad guy, so we can't be seen rewarding them, EVEN IF IT WAS FOR A GOOD DEED, they likely were just faking it anyway" which makes you QUESTION the "Hero's" morale's and honor since many of them are the "They'll redeem themselves one day".....so when they do, guess they and the world can't "practise what they preach. Also makes the "Show Mercy and giving the Villain a chance" is just a "feel good" move to LOOK GOOD, instead of BEING GOOD)

Honestly I do like this in a way in stories when the Isekai Reincarnate into a Villain starts "AFTER the Villain reputation is established" because they now have to NAVIGATE the world and story "with the annoyance of the Bad Guy Horns" as in even IF they do good, the writer of the OG story "Karma system" has to "punish them with failure or the like" because "Bad guy's HAVE to be punished" which the flaw of it is, if they were doing good that benefited someone good, the Karma system HAS TO (like an OCD maniac) make it fail because "The bad guy did it" without a DAMN CARE if it was good or evil, just the BAD GUY HAS TO FAIL (reminds me of a FEW fictional stuff I read, Like "Ladybug" where the show runner PRESENTED the FMC Bully as REDEEMING HERSELF, but then throws it out the window for no good reason, makes her a villain again and INCREASES HER BULLYING BY 20x then tries to gaslight the audience wondering WTF with "Oh she was ALWAYS evil, she was FAKING before, you're the one in the wrong for making that mistake in thinking she was redeemable" like he was FEARFUL his "change my mind" will be exposed when it was EXPOSED INSTANTLY THE SECOND HE DID THAT STUPID SHIT THEN DUG HIS HOLE BY DOUBLING DOWN AND GETTING HOSTILE WITH THE AUDIENCE ASKING FOR AN EXPLAINATION)

Interesting, wonder if the "Hiro always dies for the SMALLEST PETTIEST REASON" was just MC trying to JUSTIFY his hatred towards himself, but then I wonder why think those things! Hell the Elf Teacher Battle Maniac felt WAY different then the "she kills Hiro" like he claims she and the rest are! Or did the Misandriry he grew up with finally drive Hiro insane that he obsessed over the Yuri to assure the girls "Get happy endings" while reminding himself "he shouldn't touch them" out of the usual "Self-sacrificing Isekai" mindset, like they LOVE the characters so much that they 100% focus on getting the ENDINGS that make THEM happy, even with a few at the cost of THEM because they love them that much! Like the FL of Fiancee Observation, where she LOVES the MC, her fiancee so much when she played the game, her obsession with "being a BEAUTIFUL VILLAINESS" was to ASSURE HIS ending is picked because she knows he meets a bad end in ANY route but his own. But never realized simply ACTING as weird as she did and PROMPTING Cecil's observation of her that later morphed into falling in love with her 100% ended up "meeting the requirements to assure a better ending for all. Hell it's THANKS to her being her that Cecil ended up FIXING the results of the original story, ASSURING those in his entourage who make up the "other routes" are completely closed off from the Heroine in a GOOD MANNER (The Knight's Fiancee who is a friend of FL isn't sickly, so she's able to continue her hobbies which he shared, doing them together with him and ASSURE there is no DIVIDE the Heroine can take advantage of, her father is not evil cause her mother survived so she got a little brother from the "happy to be alive" sex they had and the one who was SUPPOSE to be her adopted brother ended up STILL finding himself WORKING under her father and apart of Cecil's entourage, and well go read that story for more! But I wonder if in my WILD ranting I was close to identifying Hiro's TRUE personality
