Not so much a loop as a detour. Lloyd was born in the fantasy world. I'm suspecting he died on the road and went to hell (obviously), and gave himself the boot to be reincarnated as Kim.. who would then become Lloyd again and boot himself from hell. After which, he goes on to rewrite the fantasy story.
