Because of Sakura - just her being there is unusual. Most boys never have a younger sister since women usually stop having kids once a son is born. So males never take care of younger siblings, instead it's always them getting cared for, confined and spoiled rotten - probably even lusted after.

Well, in a sense, if you look at society on a broader scale. Some women will have a male firstborn, but usually boys would have an older sister or two. Being an older sister isn't really rare, it's common. Yet from Sumire's perspective her statement is not wrong at all - when it comes to Sou. He will only ever have one Onee-chan. And that is an undeniable fact.
