Possibly her lazy eye developed over time. She did have a weak constitution so there might be other things that went unnoticed when she was suffering from her sickness and child birth. Remember, it's medieval Japan so anything can happen at the time when no medicine or doctors exists.

Edit: looking back at the previous chapter, she does look like she already had a lazy eye when she was introduced. Her messy look and the angles she was drawn made it look normal.
