Yeah, I noticed the wrinkles on #0 at the beginning of the chapter... I doubt he had them in his first appearance. Also, I suspect that this twist wasn't planned by that first appearance.
I think that 7 years is enough to figure it out yourself, but, in my experience of minutes since I read this, I'd say it's for aesthetic. No purpose other than looking profound, just like the way I mentioned it's been minutes since I read the stuff.
