Also, I realized in the first chapter he goes to live with "The father of my mother's niece" which would mean his mother's the young blonde.

If Ryou's the uncle, blonde's the mom, Youko is Haru, I'm going to assume the bowlcut kid is the weak-willed husband from the first chapter (the asshole woman talks about the Uncle as /brother-in-law/). Which means the only one we haven't seen from this family is Nii-san.

Which also reminds me that we haven't seen Ryou's kid at all.

EDIT: It's been a while. Ch23 Haru refers to him as Ryou.

@DirkHardpeck I don't think you have the family tree correct.

The Father of your niece is either your brother or the man of your sister. If it is your brother I think noone would bother with that complicated wording and just call them straight brother/uncle.

So my thinking is more like: The "Uncle" (very, very likely the protagonist of this flashback) is the unmarried father of the niece (Explains why they don't like him. Unmarried children weren't well received not so long ago). Which would mean his spouse/lover/? is the sister of the mother of the real protagonist, so his aunt.

It's like Oedipus. We all know what's coming, we just have to bear through watching the horrible truth as it unfolds.

I'm betting his uncle made a deal that he would have to take on her disease if it meant she would be eternally healthy, which means she's eternally at her prime. The parents probably tried to make another deal and it backfired, resulting in their deaths and their son being ostracized.

It also means she's MC's cousin once removed, which changes their dynamic a bit and makes some of their previous interactions a bit more...awkward...Though, to be fair, this always had incest undertones

I think. His uncle was in love with her. He summoned a demon and made a deal so the demon would take her form.

Since everyone is throwing in ideas, my 2 cents on the matter.

Ryou (Yuu's uncle) is quite the downer but he seems very attached to Youko (Ryou's cousin), who dialogue implies is of frail health. Since fiction taught us many, many times that love makes madmen of people, I could see Youko succumbing to whatever illness plagues her, Ryou going off the deep end due to that, and resorting to eldritch powers to save/bring back Youko. He succeeds, but she comes back "different" - as Haru, a creature of Lovecraftian lore. Wether her original personality and memories are still there one can only guess, but at the very least she knows who Ryou is and visits him at the hospital, so something of the old Youko is probably there.

Whatever the truth, I expect tragedy in the upcoming chapters.

So that girl dies of illness . The uncle looses his marbles over it, summons the demon who takes his cousins form/place. The question is why the demon continues to be into him.. Part of the contract ? and the 2nd question, how did he end up in the hospital, it sounded like he had an accident himself (accident messing with the occult) And he left teh gate open, til when Yuu met demon sis.

My original thoughts were his uncle in the hospital from the shock and terror from seeing a demon (physiological damage). But I dont know now, with how things have transgressed.
