@vexxsan I might be wrong here, but I believe the joke is that even though Yamada played the cookies off as 'everyone made them!' in reality they were 100% her personal homemade cookies and he just responded with 'well they're almost good I guess' instead of praising them. Once he realizes this, he feels like dying.

ooooooooooooooooooooooh I get it. The "I'm going to make them at home too" doesn't mean she's gonna try the same batch at her house instead. It means more she's going to make the new recipe at home as well, as in she made the previous batch there and lied about it.

No, she didn't lie. As Kyoutarou knows, they made cookies earlier in the day. When she says she'll make them at home too, she means she'll be making them at home now that she's made them at school.

The combo of her enthusiasm (which he likes) and the prospect of her making cookies for him at home is a major blow to his heart. "I'm dying!" is a common reaction to "moe". One random example:

Anyway, she's actually in a rare none flirtatious mood in this extra, in most of them she's teasing but in this one she actually just honestly wanted his opinion.

MC is, as usual, overwhelmed with various emotions, being quite unsure of what's real and what's his 'sick' imagination. Being quite perplexed of all these, he jokingly feels that it would be easier for him to just die instead of suffering from processing all these emotions. It's basically feeling like passing out anytime you're suddenly overwhelmed beyond your control.

@AWildDUK As others have mentioned, I don't think it's that she had made that batch at home and brought it to school, because then he would probably have been derailed into considering what it would mean that she'd bring cookies, to school, for him.

I think the issue is more that did take her words at face value and guessed that she was only trying to get rid of some cookies she herself wasn't particularly satisfied with. Then after his sentence she suddenly brings up a notebook and starts making a whole thing about how she's going to make more and better cookies and basically try to become a patessier or smth, and from there we come to your conclusion after all. He feels that he defo didn't live up to her expectations (or level of interest) about those cookies. He basically rained on her parade.
(He might also suddenly have coupled his cookie grading to her earlier comments about how it would have been nice if he could be a bit more positive in how he grades things. Anyway, the "complex emotions, I don't know what to do here but I think I did maybe should have done something differently" summary for his angst is probably an apt one)
