Utters have nipples, and he mentioned being able to induce lactation. Given the dairy cows normally stop producing milk a while after their calf is born and have to have another baby in order to produce again, he could make a lot of money assisting farmers if his skill works like that.

Idk how difficult it’d be but other than a guild license or so maybe he’d still need to go to med school /get some basic “training” if not existing surgeons being jealous and “blocking” him somehow versus him being a specialty consultant only called in when normal surgeries/procedures aren’t enough
