I had this idea a long time ago. Put a mini game in your vn, something pretty hard that most people would ignore because it's optional. But if you get the max score you unlock the gamer girl.

Or an random NPC without a sprite at the parc, this NPC has new dialogues every new day you talk to her, but player would assume not because who try to talk to every NPC again and again (I do)
If you talk to her everyday she get a sprite and become a route "All my life I felt invisible,

but one day you looked at me"
