Deleted chapter suddenly restored as empty entry

[Hicleric]

I was visiting that a certain shitty isekai title and noticed a staff-deleted chapter (Ch 6) had been restored as an empty chapter entry, sometime in the last couple days. The chapter was uploaded and deleted for being a fully automated TL 4 months ago, and it was only this one chapter in the bundle that was restored. I took a quick look at some other titles I knew had similar deletions but thus far only this entry seems to have been restored.

I've gone ahead and reported it as a bad entry, but I'm struck by the oddity of the occurrence, as I would have expected the system wouldn't randomly allow this to happen. Has this been observed elsewhere?
