Of course - how else can she get upset, walk away, and guilt the ML into pursuing her instead, this time around? Otherwise it'd be her running after him every time like it has been up to now, and that gets old fast, in a narrative sense.

That said - I also wonder if he's the sort of male protagonist who'd recognize her if she was just a bigger version of her 7-year-old self, without the hair change and gal makeup. Because there's plenty of romcom male leads who wouldn't.
He also seemed to remember another classmate's name that he gave a carving to, though - and FL's name is unchanged. If she wasn't really at school for him to interact with, then that's one thing, but the whole class showing up means the class should have been aware of her and her situation, and that would ostensibly include the ML.

So him not remembering her at all is....well, it sets up the dynamic between them, I guess.

If I remember correctly, he wasn't even a classmate, per se. Just someone who contributed something for local service. So for him, she was "Patient X who asked for an anime character figure," whereas for her, he was "The totally awesome kid who granted my special request for an anime character figure."

I hadn't thought about it from the time, but yeah, if her style changed this much, and he's already bad with names, then of course he wouldn't remember or connect her with her past self.

I went back to read the LN translation, and it seems that he was in fact in the same class as her in elementary school, though they still didn't really know each at the time. Since she'd been ill so often, Seika hadn't really attended class much, so she did not recognize Kousei at all when he offered to make something for her. Even her classmates knew that he was good with his hands and good with crafting, so regardless of their proximity in a classroom sense, they still didn't really interact except for his donation. So even if my original statement that he wasn't a classmate is technically wrong, the two of them only really interacted with the exchange of his "get well" gift, which is the critically important part. Shoutouts to dcdfvr for noting the discrepancy in my memory.

Didn't go back to confirm the classmate thing, so that extra degree of separation makes the chances he'd know her from Beyoncé even more remote, yeah.

But in terms of the narrative relationship between them, this also sets it up so he has to reach out and go to her, now -- up to this point, she was continuously seeking him out after their chance encounter with the bike chain that kicked things off. If she kept having to go to him, it would result in an imbalanced dynamic where they'd sit on unequal levels of importance within the plot toward one another.

This way, he also has to "opt in" to building their connection, as he has to go to her if things are to continue between them.

Ah yes, another character who suffers from one of the manga exclusive diseases. Serious enough to require a lot of time in the hospital, but it's either suddenly fatal at a dramatic moment, or completely irrelevant once the flashback ends. And it's never named so the author doesn't have to do any research or actually have a conversation about real chronic illnesses.

Narratively, that's what happens. Seika's friend Douguchi-san, who has more-or-less the full measure of what happened, reaches out to Kousei to give him Seika's home address, since Seika is too ashamed/embarrassed to explain her own actions directly. So Kousei makes the next move to resolve the confusion, and thankfully, they talk it out when they finally meet up.
