That's dark, I thought she had just died and her being there was Aina's delusion.

The rotting part of it may be due to how when a limb stops receiving blood the limb starts to suffer of necrosis (Tissue Death).

I wonder if the protagonost decision here will end up getting Aina's Mother killed later on.

You can see in some frames that Shirou made few simple tests as well, particularly patellar reflex test, where you lightly strike kneecaps to check the response. If someone thinks that they have thiamine deficiency (beriberi), it's pretty much the first thing they are going to check, since it impacts nervous system.

On the other hand CVI doesn't particularly impact nervous system directly, and result of patellar reflex test is usually normal.

So don't worry, Shirou didn't misdiagnose it just based on shallow observation of the "legs doesn't work, and swell". CVI and thiamine deficiency are very different diseases, and frankly speaking, not really that similar even with basic observation. The only similarity is swelling legs, and even those swelling looks differently, due to different primary reasons (venous system, and heart failures). Especially since he had an opportunity to touch legs in question.
