Kizaki of all people should understand that these girls are genuine psychos so while the conclusion he draws is correct, its not an easy one to draw, especially if youve been their victim for years.

Also, is the last panel implying that hotaro didnt know akira is a girl? Id assume her big brother talked about kotaro and the bullies enough but apprently not, huh.
Kinda weird that she makes all those comments in the chapter that assume akira is a girl then, but i imagine shes using gender neutral terms in the original text.

But she's surprised at the very end that Akira is a girl? I've read this over a few times and even in the next chapter she says "I thought Akira was a guy" and I still don't get this. Did the author forget halfway through or am I going insane? She know's Akira is the topic of conversation and that Akira is female, but is suddenly surprised out of nowhere? Again feel free to explain this to me as I'm drunk and probably retarded but this seems like a glaring oversight

I'm assuming it's the usual case where Japanese is gender neutral so Hotaru could be referred Akira as either a boy or a girl but the translation has to pick one. And probably in this case it picked the wrong one.
Anyway, seems like Kotaro still has some hopes to recover from his denseness. I hope this will lead to something.
